Assessment Forms

Please check the district calendar for open dates, times and locations for clergy consultations.  It is the pastor's responsibility to contact Pat in the office to schedule the consultation.  If you have any questions please contact Pat.

You will need to complete the Clergy Self Assessment and send it (preferably by email) to the District Office 1 week prior to your scheduled consultation appointment. 

The assessment forms are now available for you to download.  Just click on the form.  Be sure to save them to your hard drive before you begin filling them in!

Church Conference Reports

The following charge conference reports are available for you to download.  They are Word and PDF documents.  If you have any difficulties with these reports or if you'd prefer hard copies please contact Pat at the District Office.

Signed original Charge Conference forms MUST BE IN THE DISTRICT OFFICE 10 DAYS PRIOR to your scheduled church conference date.  Unless you use the Signature Form, which should have all necessary signatures and turned in at the time of the church conference.

Benefits Costs for 2010

Please use the following information in planning your church budgets for 2010 -

	T        The church assessment for Health Insurance for full-time pastors is  $17,520.
	Full-time pastor's personal contribution for health insurance the cost is $65/month ($780/yr).  This is to be paid as a salary reduction item from the pastor's salary.
	For a pastor to enroll his/her spouse in the health insurance the cost is $320/month ($3,840/yr)

Church's contribution to pastor's pension (Part I) calculation

For full-time pastors living in parsonages:   (salary x 1.25) x .06 =  annual payment

For full-time pastors living in own homes:    (salary + housing allowance) x .06 = annual payment

For part-time pastors (and student local pastors) in parsonages:   (salary x 1.25) x .03 =  annual payment

For part-time pastors (and student local pastors) in own homes:    (salary + housing allowance) x .03 = annual payment

For full-time pastor = $6,720; for 3/4 time pastor = $5,040; for 1/2 time pastor = $3,360; for 1/4 time pastor = $1,680.  There is no charge for lay speakers, supply pastors or retired pastors serving a church/charge.
